Dear Parents/ Carers,
Every year we need to carry out ‘data check’ to ensure that we have the most up to date information on our school system. We need to ensure that we have the correct contact details, including names of conatcts, their telephone numbers and email addresses. We also need to ensure that we have the correct address for every child and their family.

Could you please take the time to fill in this form giving us all the information required so that we can cross-reference it with what is currently on our system.
Please fill in one form for all of your children who attend Park. Please DO NOT fill in separate forms.

Dear Parents and Carers,

As part of our safeguarding policy, we need to ensure that children are only collected at the end of the school day by an adult that the parent or carer of the child has agreed with the school can collect them.

Therefore, we are asking that all parents and carers complete the following form stating the people that can collect your child at the end of the day. Please note that anyone collecting a child must be over the age of 14.

If there are any changes to what you have submitted on this form or there is an occasion where you need someone other than the people that you state below to collect your child then please contact the school office to let us know.

Recently, we were chosen to represent rockstars
across the country in BT Sports TT Rockstars Vs Celebs battle. We battled Reshmin Chowdhury, the BT sport presenter and journalist.
